genU operates a transition service in Bunbury that focuses on providing support for those living with disability and requiring mental health support, as they make the transition from hospital to home.  Our clients are all very individual and have varying needs.

For this role, we are seeking an individual experienced with complex high-care needs including behaviours. Customers have a wide range of professional supports and full training in their care plans is provided.

Key duties include:

  • Delivering a wide range of supports assisting people with disability and mental health needs as they move from hospital to home;
  • Supporting customers to engage in community-based activities;
  • Supporting person-centred plans that identify a customer’s life goals;
  • Ensuring positive behaviour support plans are followed; and
  • Administration duties, expense and mandatory reports.

What you’ll need?

  • Extensive experience in supporting customers with behavioural support plans;
  • Extensive experience working with challenging behaviours- essential;
  • Previous experience with manual handling and medication administration;
  • Significant personal care and domestic duties experience;
  • Ability to manage situations as they arise with patience and maturity;
  • Certificate in disability or mental health or higher;
  • A Valid drivers licence and a willingness to use your private vehicle (aligned with Activ’s motor vehicle allowance rebate).
