About the role

The Clinical Operations Business Partner at Activ, reports to Clinical Operations Manager and will work closely with services to ensure that clinical services align with customer goals and are delivered in accordance with NDIS Practice Standards and other compliance indicators. The role is engaged with the customer at commencement of service to when customers exit, by providing advanced, evidence-based care and support to staff on complex healthcare needs. Experience in the areas of disability, mental health or aged care will be highly regarded.

A typical day in the role may look like……

  • Deliver the highest standards of clinical care to our customers.
  • Provide clinical advice and guidance to staff.
  • Provide effective communication and guidance regarding clinical practices across the organisation.
  • Assesses, plans, implements and evaluates nursing care in collaboration with individuals to improve health outcomes for customers. 
  • In collaboration with the Clinical Operations Manager, identify and manage the follow up of clinical corrective actions.
  • Supports and liaises with customers, carers, colleagues, quality and service delivery teams, external agencies and the public and private sectors (e.g. hospitals) to provide coordinated multidisciplinary care.
  • Practices within relevant legislative and regulatory requirements in accordance with organisational policy and guidelines.
  • Participates in case conferences as appropriate.
  • Use of data and information to monitor and report performance and systems for identifying and managing clinical risk
  • Contribute to and support the implementation, application and monitoring of continuous improvement initiatives, corrective/preventative actions, or projects.

The ideal candidate will have:-

  • Demonstrated advanced clinical skills and knowledge in the delivery of evidence-based nursing care, preferably in the disability, aged care or mental health sector.
  • Highly developed interpersonal and verbal and written communication skills
  • Demonstrated advocacy for customers, families and carers within a multi-disciplinary team framework to deliver person-centred care.
  • Knowledge and understanding of continuous quality improvement, clinical audits and accreditation processes.
  • Sound knowledge of NDIS requirements and relevant legislation
  • Current AHPRA registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia
  • Valid “C” class WA drivers’ licence and vehicle
